Which sheet for which job
Meeting
Name, company, job title, email and signature: a meeting sign in sheet that doubles as the attendee list in your minutes and the address list for the follow-up.
Training session
Name, job title, email, time in, time out and signature, so the sheet shows who attended the whole session and not just the start.
Event
Name, company, email and phone for a launch, a customer evening or a trade-show booth, where the point is the follow-up list.
Visitor log
Name, company, who they are here to see, time in and time out, for a front desk, a plant or an office.
Class or workshop
Name, email and signature, short enough for a room of people to pass around in a minute.
Attendance sheet for a training or class
An attendance sheet template is a sign-in sheet with the times on it. Choose the training preset and the sheet has time in and time out next to each signature, so it records who stayed for the full session. Print one sheet per session and date it: a sheet that covers three sessions proves none of them.
Keep the signed sheet with the training material or the course record. For a recurring class, set the date and print a fresh page each time; the rows are numbered on from page to page, so a count of attendees is the last number written.

Ask only what you need
Every extra column slows the line at the door. If you will never email them, leave email off. Tell people what the sheet is for.
Put it where people enter
A sheet on a table by the door with a pen tied to the clipboard is signed by everyone. A sheet passed around halfway through misses the late arrivals.
One sheet per session
Two sessions on one sheet means nobody can tell later who attended which. Print a new page and change the date.
Follow up the same day
The sheet is the list for the thank-you, the slides and the action items. Send them while people still remember the meeting.
